How to change name with Medicare

If you have recently changed, or intend to change, your name, there are certain Government departments which you should notify. These include:
- Medicare
- Centrelink
- ATO
- Passport
- Electoral roll
You can access the first 3 (Medicare, Centrelink and ATO) through myGov but, currently, you cannot update your name with Medicare through myGov (you can with the ATO).
This blog will focus on changing your name with Medicare.
Firstly, we assume you have notified Registry of Births, Deaths & Marriages and they have processed your application and sent you your Marriage Certificate or a Change of Name Certificate confirming your new name. You will need this formal certificate for changing your name with government departments and other organisations.
There are 2 options to update your change of name with Medicare and these are by: (i) mail / email; or (ii) attend a branch.
By mail or email
The email address is: mes@servicesaustralia.gov.au and the postal address is: Services Australia, Medicare Enrolment Services, PO Box 7856, Canberra BC ACT 2610.
You will need to send certified copies of legal documents with your application (either scan and attach to your email or send a copy by post).
Attend a branch
NamesPlus recommends attending a branch and especially if your situation has changed. For example, if you were recently married and are taking your spouse’s family name, you may want a Medicare card with both your names. Or, if there are children involved, you may want to combine a family onto a single card or maybe separating onto 2 separate cards, depending on your situation.
